This role focuses on educating diverse communities about home health and hospice services, improving health literacy, and advocating for equitable access to care. The ideal candidate will be fluent in Spanish, possess strong communication skills, and be passionate about serving multicultural communities. Incorporates and demonstrates VNA’s core values of Integrity, Collaboration, Accountability, Respect, and Excellence at work and whenever representing the VNA. Responsibilities Community Outreach & Education: Develop and deliver culturally tailored health education programs on home health and hospice services. Conduct presentations, workshops, and one-on-one education sessions in both English and Spanish. Create and distribute multilingual educational materials. Build trust and rapport with diverse communities to increase awareness and engagement. Patient & Family Support. Assist individuals and families in navigating home health and hospice care options. Provide emotional support and guidance to patients and caregivers. Address cultural beliefs and misconceptions related to end-of-life care. Collaboration & Advocacy: Work closely with healthcare providers, social workers, and community organizations to ensure equitable access to services. Advocate for culturally responsive care within the organization and the broader healthcare system. Participate in community events, health fairs, and outreach initiatives. Data & Reporting Track outreach efforts and report on community needs, engagement levels, and program effectiveness. Identify barriers to care and propose solutions for improving service delivery Skills/Abilities: Excellent communication and interpersonal skills. Strong customer service skills include demonstrated ability to follow-up and follow-through on communications. Proficient in Microsoft Office with emphasis on Word, Excel, SharePoint, and PowerPoint. Willing and able to learn and become proficient in organization systems. Exceptional writing skills. Proficient in time management. Ability to multitask as necessary. Compliance with confidentiality, including HIPPA. Strong problem-solving abilities. Able to work independently, within a team, and across cross-functional teams. Demonstrated resourcefulness. Compliance minded. Meets and/or exceeds budgeted admission goals. . Qualifications Qualifications: To perform this job successfully, an individual must have the following : Fluency in English and Spanish (spoken and written). Bachelor’s degree in public health, Social Work, Nursing, or a related field (or equivalent experience). Experience in health education, community outreach, or patient advocacy, preferably in home health or hospice. Strong understanding of cultural humility and the ability to work effectively with diverse populations. Experience working with underserved or immigrant communities preferred. Knowledge of healthcare systems, home health, and hospice care preferred. Certification as a Community Health Worker (CHW) or Health Educator (CHES) is a plus. #INDSPN VNA of the Treasure Coast
